2023 Forms Will Be Added As They Become Available
1) MDWSA Contact form / Release form
Great news for 2022. Everyone that came last year (volunteers and participants) we will have this form completed with all of your contact information prefilled. All you will need to do is review and sign! For those that didn’t come last year, we will have blank forms available on-site. To minimize the confusing we are not including this form on the website.
2) USA Water Ski Waiver
Everyone will need to complete one of these forms. This includes all volunteers and participants. This form will need to be completed at each event. Two forms options, one for those under 18 and one for those 18 and above. This PDF is “fillable” on-line, you can download, fill out and print and bring with you or we will have them available on-site.
All participants will need to complete one of these forms once per year. This is for everyone that skies, rides the tube or rides in a boat. Volunteers do not need to complete. Your first event of 2022 you can print and bring or we will have on site for you to complete.
